What is Case Management?

Case Management usually refers to the efforts of a medical professional or other trained personnel to assist an individual patient in the short- and long-term treatment of an injury, disease or other medical complication. The process takes into account the unique characteristics, situation and problems facing an individual patient. Customized medication and treatment protocols are often developed by case managers to deal with specialized needs of the individual patient.
