Districts are required to annually update employees who are directly affected by the Medicare Part D program.
For districts that subscribe to a medical plan offered by SET SEG:
- SET SEG will send a Notice of Creditable Coverage to each district indicating whether their prescription drug coverage is deemed creditable. Districts are then required to provide a district-wide Creditable Coverage Disclosure Notice to all employees regarding the Creditable Coverage Program status of their prescription plan. The links above are model templates of the specific language that should be included in the notice to districts and employees.
- Update your Creditable Coverage notification each time your health plan changes or new plans are offered.
